Training For Advance Care Planning Advocates – AM Session

July 24, 2017 @ 4:00 pm - 7:00 pm PDT

Free

AM Training Description

Individuals interested in promoting advance care planning are invited to a free training program that will enable participants to become part of a community-wide movement to help individuals make their own health care choices. Today, too many people die in a way they would not choose, while leaving families feeling burdened, guilty and uncertain. Advance care planning enhances quality of life by ensuring that plans are developed to address end-of-life care decisions in accordance with personal wishes.  This training is offered free of charge as a community service in promotion of advance care planning.

Utilizing training materials developed by the Coalition for Compassionate Care of California, the program is presented by Janet Thompson, Senior Living Counselor, St. Paul’s Towers Continuing Care Retirement Community.

*AM Training Session

Designed for new advance care planning (ACP) advocates, this session includes:

  • Introduction to the concepts and benefits of advance care planning
  • Guidance on how to promote advance care planning
  • How to prepare an advance care directive
  • How to lead discussions for individuals and groups who are ready to engage in advance care planning

*Please note: You may RSVP for the AM Session, the PM Session, or both. To attend both sessions, RSVP separately for each training session.
